I don’t require support, do I still need to complete the Disability Support questionnaire?


Yes, as you have declared that you have a disability/specific need/medical condition we require the completed questionnaire to clear you for your course, even if this means stating on the questionnaire that you do not require support.  Clearly state you don't require support on the form, sign it and return it to us either by post to Disability Office, Student Support Services, Keir Hardie, Swansea University, Singleton Park, Swansea, SA2 8PP or scan and e-mail to disability@swansea.ac.uk
